Équipe santé Ontario Sudbury Espanola Manitoulin Elliot Lake Ontario Health Team (OHT) is a network of health and social service providers working together to organize and deliver services. Over time, the OHT will improve how people navigate and access the care and services they need.

The OHT does not replace the care you already receive. You will still receive services from the same providers in your community. What is changing, is that providers and organizations are working together to improve how services are organized and delivered.

We do this by working on shared priorities and working with “Persons with Lived Experience” to make improvements to programs and services. Persons with Lived Experience are the clients, residents, patients, families, and caregivers that live in and receive care in our communities. The OHT has created a Persons with Lived Experience Council to bring together the experiences and perspectives of people with lived and living experience.

The Persons with Lived Experience Council works with the OHT to make sure that the needs of the client, resident, patient, family, and caregiver are always the focus of the improvements we make.

Land Acknowledgement

Équipe santé Ontario Sudbury Espanola Manitoulin Elliot Lake Ontario Health Team works with partners situated on the traditional lands of the Anishinabek and the Peoples of the Three Fires Confederacy. The OHT recognizes these lands are subject to the Manitoulin Island Treaty (1836) and the Robinson-Huron Treaty (1850).
